9th Grade Requirements
According to the Illinois School Code, every child who enters 9th grade in Illinois shall present:
Physical: A physician licensed to practice medicine in all of its branches, an advanced practice nurse, or a physician assistant are authorized to sign health examination forms.
Immunizations: Proof of immunity against: Diphtheria, Tetanus, Pertussis, Polio, Measles, Mumps, Rubella, Hepatitis B and Chicken Pox.
Freshman students must show at least one dose of tdap (tetanus, diptheria, pertussis), Varicella (Chickenpox) Two doses of vaccine four weeks apart, or proof of immunity or date of disease is needed for all freshman and sophmores.
Diabetic Screening: The diabetic screening must be completed on the physical form for all students who require a physical.
Dental: All 9th grade students are required to have an oral health examination. The exam must be performed by a licensed dentist and turned into the school nurse by May 15th.
